May be it's some NV Optimus issue, try to set it so SVP Manager and the video player always run with NV card active.

Run SVPmark and try to pass the tests with Intel's card selected and then with NV.

Don't get me wrong but I have no idea what to do on my side to get the similar result. Unless I can do this - I can't do nothing to fix it.
"video quality" options in the driver often lead to various problems, especially on low-end cards
so yes, it's better to turn them all off
May be just one thing - ensure that ffdshow doesn't do any color conversion. So there should be YV12 input (from LAV) and YV12 output (to madVR).
By default output is set to NV12, which is not a big (or small) difference but just to make it the pure bypass filter

Those artifacts are from the not cropped video frame. Believe me, I know.
If you really are "getting the same artifacts as that other guy" then just crop that damn video at last
Otherwise just give me the information I need to try to figure out what is wrong in your case.

Performance test isn't perfect so just select profiles one step below auto-detected ones.
e.g. if you get "CPU performance level" = 3 then choose "2. Core2 Quad,..." in the list
