It looks like you are using different settings in SVP3 and SVP4.
Look at avs script in SVP3. How many threads there? For your i7-4790k it must be 15 or more.
Point that value in SVP4.

I try all HW decoders (CUVID, Quicksync, DXVA2 copy-back and DXVA2 native) in LAV Video Decoder. And I try All DXVA-compatible renderers.
I can get ideal 60 fps playback with DXVA2 native HW decoder and EVR renderer. I need to disable ffdShow in external filters tab of MPC to get work of DXVA2 native.
So, I think it is difficult to my system to decoder 4K, to copy 4K@60fps stream to RAM, and to send it back to Graphic card.
My spec:Core i5-3570K (HD Graphics 4000) + GeForce GTX 660 Ti + FullHD (not 4K) monitors.
SVP Manager stop nags me about using x64 player
Just uninstall ffdShow x64 or disable it in MPC x64.
Are you use ffdShow x64 elsewere?

64 bit support is avalable in developer builds of SVP4 for test purposes only. Maybe will come to public SVP4 with next updates but don't expect it very soon.
You must use x86 Players to work with SVP4.

Никто никого не кидал. SVP3 - бесплатный продукт, SVP4 - условно бесплатный. Первым могут пользоваться все без исключения. Вторым - только оплатившие лицензию. Либо можно бесплатно, но только урезанной по функциональности свободной версией.
Выбор за пользователем. Какую версию использовать и на каких условиях.
I use i5 because of price. And it needs 2 times less threads and 2 times less memory, so I have more chances to avoid out of memory errors when use nonstandard avisynth versions (it is needed to me for test purposes minely). Of course I have 20% less performance compare to i7. I know it and I take it into account. By the way my core i5-3570k is enough for top profile set in SVP 3 and SVP 4.
Check player version in menu about. It must be 32 bit (x86) version.
You need to reduce amount of skipped frames. Use madVR exclusive mode and ctrl-j to see dropped frames in statistics.
SVP can't get access to DirectX functions to get real refresh rate.
1. Try to close all windows applications (and games) which uses your GPU
2. Try to reinstall GPU driver (with clean install option)
And I see no GPU devices found in your system which support OpenCL. What GPUs do you have (AMD and Intel)?

To reduce artifacts you need to remove black borders from your video. You can do it with SVP autocrop option or manually.
SVP Index at 1.0 +/-0.01 is normal.
So, it is normal "teeth" 
I updated to the the current release and it was gone from the menu. Maby try the technical preview again.. is there a link to download it anymore?

It works with HD4000 and above graphics, HD2500 is too slow for FullHD video processing
It is mean "Intel HD Graphics 4000 and Intel HD Graphics 2500"

in the menu i could select HD4800 and it seemed to be working fine

It maybe mean HD4800 become compatible to SPV with new AMD drivers.
Do you see difference of GPU and CPU load when enable/disabe GPU-acceleration in SVP menu?

Пункт Avisynth управляется SVP-менеджером, если не включен, значит и повышение плавности в данный момент не включено.
Файл самый обычный, в том плане, что формат видеодорожки AVC должен воспроизводиться плеером бес всяких проблем.
Похоже, что видео декодируется без аппаратного ускорения. Иначе в окне плеера было бы указано "Воспроизведение [H/W]"
1. Если открыть файл, затем выбрать по меню плеера: Воспроизведение - Фильтры - LAV Video Dcoder, то что указано в правом верхнем углу в группе настроек Hardware Acceleration? можно скриншот
2. Какая видеокарта в компьютере?
