Two identical GPUs has different version of GPU drivers.

Another thing: DXdiag tells about Intel HD Graphics 3000. And I see no information about it in SVP logs.

Try to remove all GPU drivers (Intel and AMD) via Display Driver Uninstaller tool and install them again.

Error in the log tells about line 44 of file 407fe.avs.
Line 44 is:

    smooth=SVSmoothFps(input, super, vectors, smoothfps_params, mt=threads, src=src)

smoothfps_params starts with "gpuid:21". It is mean GPU is used with gpuid=21. And probably GPU driver could be the cause.

Forgot to mention that there's also this line: "(ffdshow_filter_avisynth_script, line 3)", which isn't listed in the crashlog for some reason.
Now it's 407fe.avs. Added new script and DxDiag in case it's going to help.

After I reinstalled everything (SVP, MPC-HC and ffdshow) I can't get SVP working again.

Getting this error:

Tried copying the Avisynth.dll to the MPC-HC folder and syswow64/system32 but still doesn't work. Tried multiple new installs of svp + mpv/ffdshow. Restarted my pc too multiple times. Resetting SVP settings doesn't work either.
